    Supplemental Information for Application for 501(c)(3) Recognition

    Dear Ms. Paladini:

    Pursuant to your request for further clafrification relating to our Application for Recognition of
    Exemption Under Section 501(c)(3) of the Internal Revenue Code which was previously submitted:

    1. Articles of Amendment. Enclosed is a copy of our Amended Articles which has been mailed to the Oregon Corporation Division. This amendment has been approved by the Board of Directors. When the Amended Articles have been filed and returned, we will send you a copy.

    2.Copy of Website. Enclosed are copies of some of the pages contained on our web site. The site is fairly extensive, and can be viewed at: http://www.winfinity.com/CFF/CFFWeb.htm. The enclosed pages should give you an idea of the general tone of our web site.

    3. Part III, Line 13 (b). All contributions were under the $283 limit other than:

         (name blocked out) - $500.00 in 1997
         (name blocked out) - $1,000.00 in 1998

    4. Legal Consultants. We do business with two attorneys. A copy of the contract with our corporate attorney, Ed Gerdes, is enclosed. Mr. Gerdis acts as our Registered Agent. Liam Sherlock provides legal services dealing with the appeal of a city council decision on rezoning and legal advice on land use planning matters. Mr. Sherlock bills us on an hourly basis. Neither attorney conducts lobbying activities for our organization.

    5. Form 5768, Election/Revocation of Election by an Eligible Section 501(c)(3) Organization to Make Expenditures to Influence Legislation. Citizens For Florence did not file this form because we do not conduct activities to influence legislation.

    6. 13.b Names and Amount of Substantial Contributor. All contributions were under the $283 limit other than as listed above (see 3. above).

    7. To my knowledge, no person involved in this organization has taken a vow of poverty.

    8. No person or entity has assigned income to the organization or made substantial contributions of money or property other than as listed above (see 3. above).

    9. Officers, Directors or Trustees related by blood or marriage: None, although I believe Directors (name blocked out) and (name blocked out) may be business partners.

    10. Written leases, contracts or agreements: None.

    11. No officers, directors, members or relatives have received or will receive any salary or any other form of payment or compensation from our organization. Occasionally, volunteers are reimbursed for actual expenses only (for example, postage, long distance charges, or copying) made on behalf of the organization.
